Patient Financial Policy

Our practice accepts most major insurance companies. It is our goal to provide fast and efficient billing as a courtesy to you. We need your help to accomplish this goal by you providing complete and accurate insurance information. All patients must complete our patient registration form and give us necessary information before seeing a provider. It is your responsibility to notify us immediately if your insurance plan or information changes. Your insurance coverage is a contract between you and your insurance carrier. Knowledge of your deductible and co-pays is your responsibility.

All patients are asked for payment at the time of service. Some procedures require payment in full at the time of service, while other appointments require only your insurance co-pay and deductible at the time of service. We accept cash, personal checks, MasterCard, Visa, Discover card and debit cards.

Insurance Plans

To assist us in filing your insurance, please remember to bring your insurance card(s) with you to your appointment. If you have a PPO, HMO, or other provider specific insurance policy please call your insurance company to see if they will cover any services performed by our providers. All insurance policies are different and it is the patient’s responsibility to know her coverage.

Please contact our business office to assist you with billing and insurance related issues. If you are not sure about our provider status with your insurance company, please check with your insurance company or with our business office.


Confidentiality of Medical Records

All of your medical records are considered confidential information. We will only release your records, or any part of them, with your written authorization. Contact our Medical Records department for any questions or concerns regarding your medical records.
